Philadelphia Archdiocesan High Schools Teachers' Labor Management Agreement
Medical Health Insurance
- Effective November 1, 2011, the contributions will be 13.5% for Personal Choice* and 11.5% for Keystone HMO.
Effective with the plan year beginning November 1,2011, all teachers will have the option of enrolling in the Blue Cross Personal Choice 25/35/70 Plan or the Keystone 10 Plan with 25/35 doctor co-pays. The teachers' monthly premium contribution shall be as follows:
*Contributions for the Personal Choice Plan are modified by the Health & Welfare Fund contributions.

Pension
Lay teachers are covered, without cost to themselves, by the Archdiocesan Lay Employees' Retirement Plan. Booklets outlining the plan have been mailed to each teacher.
Life Insurance
Group Life Insurance is provided for each lay teacher in the amount of $50,000. The cost is borne by the System. There is a voluntary Life Insurance component which teachers can opt to pay for up to $100,000 of additional benefits.
Disability Insurance
The Salary Continuance Plan (Long Term Disability Plan) pays a monthly benefit of 60% of gross salary. The plan is designed to replace income lost during a period of total disability due to covered injury, sickness or pregnancy. One-half of the cost of this plan is borne by the System. The teacher bears the remainder of the cost of the plan.
Dental Program
All full-time permanent lay teachers and their dependents are eligible for coverage under the Association Health and Welfare Fund's Dental Plan through United Corcordia. The plan pays the usual, customary and reasonable charges for dental services up to a maximum of $1,000 per person per year. The plan includes major services such as root canal. Periodontics are covered at 50% of UCR. Dentures, caps and crowns, and orthodontia are not covered under the plan. Specific coverages and exclusions are listed in the plan booklet.
