Philadelphia Archdiocesan High Schools Teachers' Labor Management Agreement
Medical Health Insurance
For plan years from November 1, 2019 through October 31, 2022, all teachers will have the option of enrolling in the Blue Cross Personal Choice Plan or the Keystone HMO Health Plan which includes a $100 per individual deductible and $200 per family deductible.
A Personal Choice High Deductible Health Plan will also be offered. The System shall pay $500 individual/$1,000 family toward the annual deductible of $1,500 individual/$3,000 family for any teacher who selects the HDHP.
The teachers' monthly premium contribution shall be as follows:
- The contributions will be 15% for Personal Choice, 12% for Keystone HMO, and 10% for Personal Choice High Deductible Health Plan (HDHP).

Lay teachers hired prior to 2014-2015 school year were covered without cost to themselves, by the Archdiocesan Lay Employees' Retirement Plan. Booklets outling the plan should have been mailed to each teacher.
Beginning with the 2014-2015 school year, all teachers are covered, without cost to themselves, by the Archdiocesan Defined Contribution Retirement Plan. The carrier is Vanguard.
Group Life Insurance is provided for each lay teacher in the amount of $50,000. The cost is borne by the System. There is a voluntary Life Insurance component which teachers can opt to pay for up to $100,000 of additional benefits.
The Salary Continuance Plan (Long Term Disability Plan) pays a monthly benefit of 60% of gross salary. The plan is designed to replace income lost during a period of total disability due to covered injury, sickness or pregnancy. One-half of the cost of this plan is borne by the System. The teacher bears the remainder of the cost of the plan.
All full-time permanent lay teachers and their dependents are eligible for coverage under the Association Health and Welfare Fund's Dental Plan through United Corcordia. The plan pays the usual, customary and reasonable charges for dental services up to a maximum of $1,000 per person per year. The plan includes major services such as root canal. Periodontics are covered at 50% of UCR. Dentures, caps and crowns, oral surgery and orthodontia are not covered under the plan. Specific coverages and exclusions are listed in the plan booklet.
